Both reports come from an inspector at the house.
Florida carriers ask for two inspections that touch the roof: the 4-point and the wind mitigation form.
A 4-point looks at four systems, with the roof listed beside wiring, plumbing and HVAC. Most Florida carriers want one before they write or renew on any roof beyond about 15 years.
The wind form, OIR-B1-1802, scores seven features, roof deck attachment and roof geometry among them. Your insurer applies the credits once a qualified inspector completes it.

No Florida statute sets a maximum roof age, and the inspection request comes from your carrier rather than the state. Most carriers here ask for a 4-point once the roof is past about 15 years, before they will write or renew. Each carrier sets its own appetite; ask yours which report it needs.
It can. Since the 2022 reforms (SB 2-D), a carrier generally may not turn down or drop a homeowners policy over shingle age past 15 alone, provided an authorized inspector documents 5 or more years of remaining useful life. That inspection report is the document your insurer acts on.
Yes. Roof geometry is scored on OIR-B1-1802, and a hip roof earns a better mark than a gable roof. Houses built under the 2002 Florida Building Code or newer tend to qualify for the credits, while older ones often get there after modest retrofits.
